Protecting Your Reproductive Health: A Look at Problems We Help Manage

Women's health is a complex and sensitive matter that requires compassionate and expert care. The office of Dr. Herrmann provides comprehensive gynecological services for women of all ages, from adolescence to menopause. Dr. Herrmann and his staff are committed to providing supportive, informative, and personalized care to their patients.


Annual Exams and Office Visits

Annual exams and office visits are essential for maintaining proper health and detecting any potential health issues. At Dr. Herrmann's office, all annual exams and office visits include a thorough check-up of height, weight, BMI, B/P, and LMP, along with any necessary recommendations based on the patient's age.


Adolescent and Young Adult Patients: For patients aged 13-20, The Herrmann Practice provides a consultation to discuss menstrual cycles, HPV vaccinations, STIs, birth control options, and exams when necessary. Dr. Herrmann and his nurse practitioner provide age-appropriate guidance on reproductive health issues and ensure that young women receive the information they need to make informed choices.


Adult Patients: For patients aged 21-29, Dr. Herrmann recommends adding a breast and pelvic exam, and a pap cytology, with HPV (Human Papillomavirus) when indicated. For patients aged 30-65, HPV testing is added to the pap smear every 3-5 years.


Birth Control Options

Dr. Herrmann provides a wide range of birth control options, including oral contraceptives, birth control patches, contraceptive vaginal rings, Depo Provera injections, and long-acting reversible contraceptives (LARC), such as Nexplanon and IUDs. Dr. Herrmann works with patients to determine the best birth control method based on their individual needs and preferences.


  • For oral contraceptives are daily pills like LoLoEstrin Fe, Nextillis, Junel, and Cryselle. Generally. insurance will cover the costs.
  • Birth Control Patches such as Twirlla or other generics need to be changed weekly for three weeks, and then off for 4-7 days.
  • Contraceptive Vaginal Rings like NuvaRing need to be changed monthly, but there are year-long options like Anovera.
  • Depo Provera injection for every 12 weeks
  • Long Acting Reversible Contraceptives (LARC) are non-surgical implants like Nexplanon (lasts 3 years) for progestogen only, and IUD inserts like Kyleena (lasts 5 years), Mirena (lasts 8 years), or a non-hormonal copper IUD, Paragard (lasts 10 years).


Patients on hormonal birth control may experience breakthrough bleeding, which can usually be treated with a cycle or short course of oral contraceptives. Patients should be aware that breakthrough bleeding is common and not a cause for alarm. Patients not on hormonal medications should have a menstrual period every 21-35 days and excessive flow may require further evaluation for uterine pathologies. Heavy or prolonged periods can be treated hormonally or surgically, including the use of hormonal IUDs or endometrial ablation.


STI Testing

STI testing is offered to all sexually active patients and recommended according to CDC guidelines for Chlamydia, Gonorrhea, Syphilis, and HIV. Dr. Herrmann's team also tests for ulcerated vulvar lesions and provides partner treatment per the patient's request.


Vaginal Infections

Dr. Herrmann's office provides testing and treatment for common vaginal infections, such as bacterial vaginosis, monilial vaginitis (yeast), and trichomonas. Dr. Herrmann and his team work with patients to provide the most effective and appropriate treatment for their specific condition.


Bacterial Vaginosis (BV) is the most common, characterized by a fishy odor and whitish/grayish discharge. Treatment options include metronidazole orally or generic metronidazole gel. Recurrent/chronic cases may require longer treatment with Boric acid capsules. Monilial vaginitis (yeast) causes pruritus and a cottage cheese-like discharge, treated with Diflucan. Trichomonas causes a bubbly discharge, treated with oral metronidazole. Partners can be treated if desired.


Menstrual Disorders

The Herrmann Practice provides guidance and treatment for menstrual disorders, including breakthrough bleeding for patients on hormonal birth control, heavy flow, and prolonged periods. Dr. Herrmann and his team also evaluate uterine pathologies, such as uterine fibroids and endometrial polyps, and provide hormonal or surgical treatment, including hormonal IUDs and endometrial ablation.


Menopausal Patients

Menopause typically occurs between 45-55 years of age and is characterized by irregular menstrual cycles and hot flashes. After 12 months of no periods, any bleeding should be evaluated. Hormonal therapy (HT) can be considered for patients experiencing severe symptoms, with estrogen available in various forms and deemed safe for most patients. Progesterone is required for patients with a uterus. HT can provide additional benefits such as preventing bone loss and improving skin and mood. Bioidentical patches and oral progesterone are recommended as the safest HT options.


Vulvar Pruritus

Vulvar itching is a common condition that can be caused by a variety of factors such as yeast or fungal infections and chemical irritants. Vulvar skin is particularly sensitive, especially for those with lighter skin. It's important to avoid fragrance-containing products and laundry detergents, as they can exacerbate itching and burning. A steroid ointment can provide relief and help break the itch-scratch cycle. Over-the-counter products like Vagisil may worsen symptoms. Don't hesitate to seek help and treatment from a healthcare provider for vulvar itching. We are here to help diagnose and treat these issues, and there is no need to feel embarrassed or miserable.


Pap Tests

Pap tests are an effective screening method for cervical cancer and can detect abnormal cells long before they become cancerous. The test also detects vaginal infections and STIs. For patients over 30, the test is used in combination with an HPV test. The HPV vaccine is safe and effective in preventing HPV-related cervical cancer, but should be given before exposure to the virus. An abnormal Pap test does not necessarily mean cancer and requires further evaluation in the office.


Breast Lumps

Finding a breast lump can be extremely alarming, but most lumps are not cancerous. We can see any patient who feels something abnormal in their breasts on the same day. After an exam, we can determine if further evaluation with a mammogram or ultrasound is necessary. We recommend annual mammograms starting at age 40, as they are the best method of screening for breast cancer.


Urinary Tract Infections

Urinary tract infections (UTIs) are common in females and are caused by bacteria from the rectal area. Symptoms include burning with urination, frequent urination, and a constant urge to urinate. It's important to empty the bladder before and after intercourse. Untreated UTIs can develop into kidney infections and require hospitalization. We can usually treat UTIs in the office. It’s best to obtain a fresh specimen in the office for culture.


Urinary Incontinence

Urine leakage can occur in women of all ages and may worsen with childbirth and aging. It can be caused by changes in anatomy or weakening of the bladder's support. Treatment options include exercises, medication, and surgery. Don't suffer in silence, we can help. Referral to a urogynecologist may be necessary in some cases.


Weight Loss Program

We provide diet and exercise counseling as well as medications for weight loss.
